I understand that the consulting doctor and the Sanan Relief team will rely on this information for issuing medical advice or prescription through telemedicine as per the Telemedicine Practice Guidelines, 2020 issued by the Medical Council of India. I acknowledge that Sanan Relief and its associated doctors shall not be held liable for any adverse effects, complications, or outcomes arising due to misinformation, omission of relevant medical history, failure to follow the prescribed dosage or instructions, or self-medication. I also understand that my personal and medical data shall remain confidential and handled in accordance with the Information Technology (Reasonable Security Practices and Procedures and Sensitive Personal Data or Information) Rules, 2011. By submitting this form, I provide my explicit consent to receive medical consultation and prescription from Sanan Relief through telemedicine under applicable Indian laws.

Sanan Relief respects your privacy and is committed to protecting your personal and medical information. All data collected through this platform – including your name, contact details, medical history, and consultation records – is treated as Sensitive Personal Data or Information (SPDI) under the Information Technology (Reasonable Security Practices and Procedures and Sensitive Personal Data or Information) Rules, 2011.

Your information will be used solely for providing telemedicine consultation, prescription, follow-up, and related healthcare services as permitted under Indian law. It will not be shared, sold, or disclosed to any third party without your explicit consent, except where required by law or regulatory authorities.

By continuing to use this service, you consent to the collection, storage, and lawful use of your personal and medical information by Sanan Relief for the purposes stated above.

Terms of Telemedicine Consultation

  1. Nature and Scope of Service: Telemedicine services offered through Sanan Relief are intended to provide medical advice, consultation, and prescription support remotely using secure electronic communication. These services are provided in good faith based on the information voluntarily disclosed by the patient and do not replace an in-person clinical examination where such examination is necessary.
  2. Establishment of Doctor-Patient Relationship: By consenting to teleconsultation, the patient authorizes Sanan Relief to connect them with registered medical practitioners for advice or prescription. This creates a valid doctor-patient relationship under the Telemedicine Practice Guidelines, 2020.
  3. Responsibilities of the Patient: The patient agrees to provide accurate, complete, and truthful medical information. Any misrepresentation or omission may affect medical advice and absolves Sanan Relief and its doctors of related liability. Patients must follow prescribed dosages and instructions. Any self-medication or alteration without consultation is at their own risk.
  4. Responsibilities of the Doctor: Consulting doctors shall act per the Telemedicine Practice Guidelines (2020) and the Code of Medical Ethics (2002). Prescriptions will be issued only after proper assessment and within permissible telemedicine limits.
  5. Confidentiality and Data Protection: Sanan Relief ensures that all patient data and records are securely stored and accessible only to authorized personnel, managed per the IT SPDI Rules, 2011.
  6. Limitation of Liability: Sanan Relief and its doctors are not liable for indirect or consequential damages arising from miscommunication, technical errors, or patient non-compliance. This includes self-medication, misuse of prescribed drugs, or concealment of relevant medical history.
  7. Consent and Governing Law: By proceeding with teleconsultation, the patient confirms having read, understood, and accepted these terms voluntarily. These terms are governed by Indian law and subject to the exclusive jurisdiction of courts at Indore, Madhya Pradesh.
